If I add an app to a board and as part of the app I change the items displayed on the board, will those changes show up right away in the UI or will a page refresh be required?

Welcome to the community. If everything works as expected there is no need for a refresh when updating through the API. However, I have seen situations that the monday “pusher” is not working correctly and that a refresh solves it. But again: normally you won’t need a refresh.
